Remit completed dog license application form to the Town Clerk's office, either in person or by mail with appropriate fee (check with Town Clerk's office for local fee amount); spay/neuter certificate (if applicable); proof of rabies vaccination signed by a licensed veterinarian; and if applicable, proof of exemption status.
